Step 1: Obtain the yellow page of the JW form
1. If you still have the yellow page of the JW form you received at enrollment,the validity of which should be able to cover the validity of residence permit that you plan to apply for , you can use that yellow page directly.
2. If you have already used or missed the yellow page of the JW form you received at enrollment or the validity of that yellow page cannot cover the validity of residence permit that you plan to apply for, you should apply for a new JW form according to the following instructions:
A. If you are a self-supporting undergraduate/language student from the Overseas Education College, please proceed to B207, OEC on Xiang'an campus to apply for JW form;
B. If you are not a self-supporting undergraduate/language student from the Overseas Education College, please proceed to Student Affairs Hall (Counter 5, 1F, Jiageng 3, Siming Campus) to apply for Certificate for JW Form Application. (Please note that graduating students applying for study extension should bring to Student Affairs Hall the Proof of Study Extension: print the approved application webpage on the system of the Graduate School or ask the college office for it). Go to the Admissions Office (3F, Jiageng 3 Bldg., Siming Campus) with the Certificate for JW Form Application and apply for new JW form.
***The Admissions Office only accepts JW form application on the 1st, 11th and 21st of every month.(Weekends and holidays excluded) Since it takes at least 2 weeks for the approval of new JW form, we advise students who need to extend the residence permit apply at least 1 month in advance to avoid overstaying your residence permit.
Step 2: Prepare the following documents：
o Accommodation Report
o Passport, copy of passport and residence permit page
o Yellow page of the JW form
o 2 2-inch photos (recent, white background)
o ***Introduction Letter（Students at the Overseas Education College only, available at B207, #2Bldg. Xiang'an Campus）
o ***Proof of Study Extension（Graduating students applying for study extension ONLY,print the approved application webpage on the system of the Graduate School or ask the college office for it)
Step 3：Bring all the required documents to Student Affairs Hall (Counter 5, 1F, Jiageng 3, Siming Campus) to apply for university documents.
Step 4：Proceed to PSB to apply for new residence permit with all the documents above.
